Requirements

• Master’s Degree in Social Work (MSW) required.
• Active LGSW or LISW license in Minnesota.
• At least 1 year of clinical Social Work experience.
• A passion for independent, relationship-based care.

Responsibilities

• Provide in-home visits supporting patients and families.
• Deliver counseling, care planning, and crisis intervention.
• Connect patients to community resources and support systems.
• Collaborate with an interdisciplinary care team.
