About The Position

The Social Worker-MSW Urgent Care serves as a key member of the patient care delivery team. Identifies patient and family psychosocial and environmental needs related to urgent treatment and discharge. Works collaboratively with other members of the interdisciplinary team in developing and implementing a comprehensive, integrated discharge plan. Makes appropriate referrals for community services; may provide emotional support and counseling services, crisis intervention. Assesses and intervenes in situations involving child or elder abuse/neglect, domestic violence. Provides care and services to all patient groups: Neonatal, infant, pediatric, adolescent, adult and elderly. Performs other duties as needed.

Requirements

  • Master's Degree in social work required.
  • Minimum one year of experience in clinical setting required.
  • Able to speak, read and write legibly in English.
  • Use computer, printer, and software programs necessary to the position, e.g., Word, Excel, Outlook, PowerPoint, electronic medical record, electronic event reporting program.
  • Relate and communicate positively, effectively, and professionally with others.
  • Be assertive and consistent in following and/or enforcing policies.
  • Work calmly and respond courteously when under pressure.
  • Lead, supervise, teach, and collaborate.

  • Work independently with minimal supervision.

  • Previous urgent care experience preferred.
  • Spanish preferred.
