Pediatric Specialty Care is currently seeking a compassionate Social Worker that will serve on an inter-disciplinary team that provides care coordination, discharge planning and support to individuals and families with necessary resources. This role is an integral part of the IDT team and facilitates coordination of information between internal members and external agencies such as regulatory agencies, Children and Youth Services, and funding sources. We are seeking a Social Worker to assist our Point Pleasant, Doylestown, and Quakertown programs. The Social worker will act as part of the interdisciplinary team to achieve the best outcomes for the individuals we serve. The Social Worker Position Overview: Will assists families with admission, crisis management, resolving personal, financial and environmental issues. Maintains regular and frequent contact with families, caseworkers, school staff, and therapists as necessary. Completes insurance update and COMPASS renewals of benefits Participates in bi-weekly and ad-hoc social services calls.
