This role involves working collaboratively within an interdisciplinary team to assist patients and families facing complex psychosocial needs. Interventions may include alternate site coordination, discharge planning, addressing abuse and neglect, adoption, substance use disorders, end-of-life decisions, providing referral resources, bereavement support, adjustment counseling, facilitating guardianships, and managing behavioral health concerns, including psychiatric transfers. The Social Worker will conduct psychosocial assessments, communicate with external sites and agencies, document assessments and plans in the electronic medical record, provide consultation to the healthcare team, maintain knowledge of community resources, participate in stakeholder committees, and stay current with federal and state regulations. Short-term therapeutic support will be provided as appropriate.
