Works as part of an interdisciplinary team to provide outreach services to individuals in the community who have difficulty accessing services with serious substance use disorders and other complex medical and psych-social needs such as mental health and/or primary care needs. Provides comprehensive Social Work assessments, connects clients with health care and community services, develops, implements and adjusts individualized treatment plans in collaboration with internal team, affiliated prescribers and other treatment team members and provides counselling according to a variety of models. Participates in team case conferences/meetings to coordinate and manage client care issues, aids clients access to needed services, establishes and maintains relationships, liaises with staff of emergency rooms, inpatient psychiatric units and other resources and collaborates and consults with colleagues. Participates in team meetings, continuing education, conferences and/or corporate initiatives and engages in on-going team planning, development, evaluation and quality improvement activities. Provides clinical and consultative Social Work services, including outreach and brief counselling. May act as a Designated Responder under the Adult Guardianship Act.
