About The Position

Humber River Health is seeking a Social Worker to join their Mental Health & Addictions Program. As North America's first fully digital hospital, Humber River Health focuses on using technology and clinical expertise to improve patient care and reduce inefficiencies. The role involves working with patients in an inpatient adult mental health setting, utilizing a variety of therapeutic modalities and collaborating with a multidisciplinary team. The hospital is affiliated with the University of Toronto and Queen’s University and is dedicated to becoming a community academic hospital.

Requirements

  • Master of Social Worker (MSW) obtained from an accredited School of Social Work.
  • Current membership with the Ontario Association of Social Workers (OASW) and Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Demonstrated skills in working with a broad range of client groups and their families.
  • Demonstrated skills in individual and group counselling/psychotherapy.
  • Knowledge and experience with different models of assessment and treatment for problem substance use, addiction and concurrent disorders.
  • Demonstrates understanding and incorporates trauma informed approaches.
  • Excellent organizational skills with demonstrated flexibility and adaptability to work in a fast paced/changing environment.
  • Ability to work independently as well as in a team environment.
  • Demonstrated excellent interpersonal and communication skills.
  • Ability to respond rapidly and decisively to crisis situations.
  • Excellent decision making skills.
  • Excellent knowledge of community resources.
  • Demonstrated ability to prioritize a consistent and varied workload.
  • Demonstrates cross cultural awareness and sensitivity.
  • Good word processing skills, including MS Office.
  • Excellent attendance and discipline free record required.

Nice To Haves

  • Certification in the prevention and management of aggressive behaviour is an asset.
  • Two (2) years’ experience in mental health and/or addiction facility or setting preferred.
  • Knowledge of and experience in CBT, DBT, and Motivational Interviewing a definite asset.
  • Experience in withdrawal management services and knowledge of early recovery treatment models is an asset.
  • Knowledge of a second language an asset.

Responsibilities

  • Completes screening and a detailed addiction assessment for all referred patients.
  • Provides psycho-social assessment and therapies with individuals, families and groups using a variety of psychotherapeutic modalities (supportive, dynamic, interpersonal, counselling and solution-focused) to promote an optimum level of functioning.
  • Supports the patient during all phases of recovery from usage to abstinence.
  • Using a holistic approach provides education and consultation services to patients and their families to promote an optimal level of functioning.
  • Develops and coordinates the discharge plan in conjunction with the treatment team including connection/referral to appropriate internal and/or community resources.
  • Completes patient documentation, workload measurements and data requirements in accordance with Hospital, Ministry of Health, CLHIN and departmental standards.
  • Responds to clinical crisis in an effective and efficient manner.
  • Acts as resource of discipline-specific expertise to other members of the treatment team.
  • Works in a manner to ensure safety to oneself, clients, colleagues and the work environment.
  • Participates in reflective practice as per identified college standards.
  • Other duties as assigned.
