About The Position

Tallahassee Memorial Healthcare (TMH) is a private, nonprofit community-based healthcare system providing care to a 22-county region in North Florida and South Georgia. The Social Worker/MSW in Case Management works as a multi-disciplinary team member to assess and treat patients/clients/families facing emotional, behavioral, and health problems. This role involves providing comprehensive social work services, interviewing patients and families, evaluating social and emotional factors, identifying psychological needs, coordinating services, and facilitating individualized case management. Key responsibilities include providing clinical counseling and individual therapy, assisting in treatment planning, offering direction to the clinical team, and delivering crisis intervention services. The position also involves advising family caregivers, providing case and care management, offering information and counseling, making referrals, maintaining patient contact, and potentially executing discharge planning. The role may include participation in social and health programs and travel to patients' homes. Requirements

  • Master’s degree in a mental health specialty (e.g., Master of Social Work, Master of Mental Health, Master of Mental Health Counseling, or Master of Marriage and Family Therapy) from an accredited university.
  • One (1) year of relevant job-related post-master’s degree work experience in a supervised clinical setting (required if providing home health care).
  • A valid state driver license (required if providing home health care).
  • Must be at least twenty-one (21) years of age and have a minimum of two (2) years of licensed driving experience (required if providing home health care).
  • Must provide a seven (7)-year driving record from the Department of Motor Vehicles (DMV) prior to employment (required if providing home health care).
  • Must have received no more than two (2) minor violations in the past three (3) years and zero (0) major violations in the past five (5) years (required if providing home health care).
  • Use of personal vehicle(s) required (if providing home health care).
  • Vehicle owner must maintain adequate insurance coverage for personal vehicle(s) used in conjunction with duties (if providing home health care).
  • Successful completion of a drug screen, background check, reference verification, health assessment, and credential/license verification.

Responsibilities

  • Works as a multi-disciplinary team member.
  • Assesses and treats patients/clients/families with emotional/behavioral/health problems.
  • Provides social work services to patients/clients/families with behavioral, social, financial/economic, and environmental problems.
  • Interviews patients/clients and their families.
  • Evaluates, and understands the social and emotional factors underlying patients’/clients’ illnesses; identifies their psychological needs, goals, and treatment preferences; coordinates a range of services; and facilitates individualized case management.
  • Provides clinical counseling and individual therapy to help patients/clients/families make positive behavior changes.
  • Assists patients/clients in treatment planning.
  • Provides direction to clinical team for implementing treatment plans.
  • Provides crisis intervention services to patients/clients and families.
  • Advises family caregivers; provides case and care management or interventions designed to promote health/well-being and prevent disease.
  • Provides patients/clients/families with information and counseling; makes referrals for other services.
  • Maintains contact with patients/clients/families to assist in adjusting their needs through use of medical facilities or referral to outside supportive agencies.
  • May execute discharge planning responsibilities.
  • Participates in social and health programs in the region/community; may assist/participate in planning supportive events.
  • May travel to patients’/clients’ homes to render services.
