The listed pay range or pay rate reflects compensation for a full-time equivalent (1.0 FTE) position. Actual compensation may differ depending on assigned hours and position status (e.g., part-time). Pay Range: $67,225.60 - $87,838.40 Scheduled Weekly Hours: 40 Position Overview ESSENTIAL FUNCTIONS Conduct comprehensive psychosocial assessments and mental status examinations to formulate DSM-5-TR diagnoses and treatment plans Provide evidence-based counseling and psychotherapy to individuals, groups and families, using advanced expertise in medical/mental health diagnoses, chronic illnesses and medications to meet the needs of a diverse patient population Identify patient strengths, coping skills and needs in collaboration with families and interdisciplinary teams Deliver immediate crisis intervention services and represent patient rights and needs to providers and the community Maintain accurate medical records, progress notes and treatment plans in accordance with clinic protocols and regulatory agency requirements Monitor patient outcomes and registries to develop workflows and performance initiatives based on quality metrics and best practices Continually review and implement regulatory requirements to ensure all social work services and documentation meet mandatory standards Lead planning meetings, case conferences and care team huddles and provide expert consultation on how psychosocial factors impact patient health and treatment compliance Serve as a primary liaison between outside agencies, hospital departments and the medical team Maximize staff scheduling and assist with the recruitment, hiring and onboarding of new clinical social work personnel Meet regularly with LSW and CSW staff to identify and resolve operational issues, providing recommendations to the leadership team Coordinate staff training and performance improvement initiatives to increase clinical awareness of biological, psychological and social issues Perform other duties as assigned

Requirements

  • Master of Social Work (MSW) required.
  • Five years of independent clinical social work experience in a mental health or hospital-based setting performing medical crisis counseling and discharge planning.
  • At least two years must also include developing, implementing and coordinating clinical programs and patient support services.
  • AHA BLS - Healthcare Provider
  • Licensed Clinical Social Worker required

Responsibilities
