About The Position

Kline Galland Home is actively seeking a Social Worker (Discharge Planner) and includes a $5,000 Sign on Bonus for a well-established team on our Transitional Care Unit (TCU) where patients receive short-term, post hospitalization rehabilitation. The social worker will play an active role as a member of an interdisciplinary team consisting of nurses, doctors, Physical therapists, Occupational therapists, Speech language therapists, and other medical professionals. The primary role of the social worker is to create a safe discharge plan in coordination with the patient, family, and interdisciplinary team. Completion of all required assessments, insurance reviews, and medical chart documentation Frequent interdisciplinary communication and meeting coordination Caseload management from admission to discharge Maintain effective, timely communication with patients and their families Understand and meet all government requirements for documentation

Requirements

  • Degree in Social work or related field of study
  • 2 years' experience in Human Services or applicable field
  • Prior experience in activities a plus
  • Ability to manage moving priorities each day
  • Strong verbal and written communication skills
  • Computer skills for charting and documentation
  • Strong team player with the ability to have FUN!
