About The Position

We are seeking a dedicated Discharge Planner – LPN or Social Work to join our long-term care team and play a vital role in ensuring safe, smooth, and person-centered transitions from admission through discharge. Key Responsibilities of a Social Worker: Manage an assigned caseload from admission through discharge Develops discharge plans and documents interventions and pertinent information in medical records on a timely basis Conduct psychosocial assessments and develop individualized care and discharge plans Provide counseling, emotional support, and strong advocacy for residents and families Collaborate with the interdisciplinary team, participate in care conferences, and facilitate family meetings Connect residents with community resources and arrange appropriate post-discharge services Provide crisis intervention and address behavioral or emotional changes Serve as a resource for staff and families and participate in ongoing professional development If you are passionate about advocating for residents, supporting families, and collaborating with an interdisciplinary team to enhance quality of life at every stage of care, we invite you to join our team!

Requirements

  • Previous experience in long-term care, geriatrics, or a related healthcare setting
  • Knowledge of discharge planning processes and community resources
  • Strong assessment, documentation, counseling, and crisis intervention skills
  • Excellent communication and interpersonal abilities
  • Commitment to person-centered care and resident advocacy
  • Must have a LSW, LSWA or LPN license
