Requirements

Previous experience in long-term care, geriatrics, or a related healthcare setting
Knowledge of discharge planning processes and community resources
Strong assessment, documentation, counseling, and crisis intervention skills
Excellent communication and interpersonal abilities
Commitment to person-centered care and resident advocacy
Must have a LSW, LSWA or LPN license

Responsibilities

Manage an assigned caseload from admission through discharge
D evelops discharge plans and documents interventions and pertinent information in medical records on a timely basis
Conduct psychosocial assessments and develop individualized care and discharge plans
Provide counseling, emotional support, and strong advocacy for residents and families
Collaborate with the interdisciplinary team, participate in care conferences, and facilitate family meetings
Connect residents with community resources and arrange appropriate post- discharge services
Provide crisis intervention and address behavioral or emotional changes
Serve as a resource for staff and families and participate in ongoing professional development
