About The Position

Join the compassionate team at Trinity Health St. Mary’s Hospital as a Medical Social Worker. This role involves utilizing clinical social work skills to make a meaningful difference in the lives of patients and their families within an acute hospital setting. You will be responsible for conducting psychosocial assessments, providing brief solution-focused counseling, and developing collaborative treatment goals. Your work will focus on helping patients overcome barriers, access essential community resources, and ensure seamless continuity of care across various inpatient and outpatient settings, including ICU, med/surg, Emergency Department, mother/baby, and the psychiatric medical unit. The position is part-time with a rotating shift.

Requirements

  • Master’s Degree in Social Work or a related field from an accredited institution.
  • Must be eligible for licensure as a Limited Licensed Master Social Worker (LLMSW) or Licensed Master Social Worker (LMSW) by the Michigan Board of Social Work.
  • Minimum of 2 years of social work experience in an acute inpatient setting.
  • Strong psychosocial assessment skills and clinical judgment.
  • Ability to navigate complex family dynamics and high-acuity situations.
  • Excellent crisis support and problem-solving abilities.
  • Outstanding interpersonal and empathetic communication skills.
  • Proficiency in engaging patients and care teams in effective treatment planning.
  • Knowledge of community resources and continuity of care principles.
  • Accurate, timely documentation in medical records and reporting systems.

Responsibilities

  • Conduct comprehensive psychosocial assessments for patients and families.
  • Develop treatment plans that address emotional, social, and resource-related needs.
  • Provide brief, solution-focused counseling and facilitate discharge or care planning.
  • Advocate for patients and coordinate referrals to community-based services.
  • Collaborate effectively with interdisciplinary care teams.
  • Document all assessments, plans, interventions, and outcomes clearly and promptly.
  • Meet departmental productivity expectations and performance standards.
