Requirements

  • Masters’s degree required
  • BLS certification and Crisis Intervention Training required within 90 days of hire
  • State of North Carolina Licensed Master’s Social Worker (LMSW)
  • 2 years related work experience
  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action.
  • Able to communicate effectively in English, both verbally and in writing.

Responsibilities

  • Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
  • Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
  • Participate in case reviews to evaluate case management and progress.
  • Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
  • Assists with discharge planning and processes.
  • Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
  • Performs Medicaid screenings as indicated for NHP.
  • Assist patients with Safety Net applications when needed.
  • Monitors and evaluates the quality and appropriateness of social services on the inpatient behavioral health units in accordance with accepted standards of practice and established policies and procedures.
  • Conducts a bio-psycho-social assessment of the patient and participates in the treatment planning process, with ongoing reassessment and evaluation.
  • Formulates individualized measurable goals for the patient.
  • Participates in discharge planning, arranging for follow up care, continuity of care and coordination with outside agencies.
  • Uses various modalities such as group therapy, individual therapy to treat the patient and assist families
  • Participates in utilization management activities as needed to ensure proper payment for services.
  • Completes required documentation within required time frames.
