About The Position
Atlantic Health System is seeking a Per Diem Social Worker for Case Management at Chilton Medical Center. The role involves providing Social Work coverage to facilitate discharges, meeting with patients and families to assess and provide discharge planning support, resources, referrals, and transfers to Extended Care and inpatient psychiatric facilities. The Social Worker will collaborate closely with the Healthcare Team to implement the plan of care, obtain necessary insurance authorizations, and communicate with community resources and area facilities. Phone, Fax, and Electronic documentation are required.
Requirements
- LSW (Licensed Social Worker)
- Per Diem status
- Ability to provide Social Work coverage to facilitate discharges.
- Ability to meet with patients and families to assess and provide discharge planning support, resources, referrals, and transfers to Extended Care and inpatient psychiatric facilities.
- Ability to work closely with the Healthcare Team to implement the plan of care.
- Ability to obtain insurance authorizations as required.
- Ability to communicate with community resources and area facilities.
- Proficiency in Phone, Fax and Electronic documentation.
