Requirements

• Master's Degree in Social Work from an accredited school
• Upon hire: Washington Clinical Independent Social Worker Associate License
• 1 year of experience in the areas of acute medical or mental healthcare in an inpatient or outpatient setting

Nice To Haves

• ACM/CCM certification
• 1 year of experience with organ transplant

Responsibilities

• Providing a full range of social work services including but not limited to psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral.
• Partnering with the patient and his or her support system, as well as interdisciplinary teams, both internal and external to the organization, to improve patient care through the effective utilization and monitoring of health care resources.
• Assuming a leadership role to facilitate the achievement of patient goals and desired clinical, financial, and resource outcomes.
• Addressing and advocating for patients' social and emotional needs.
