About The Position

NeighborHealth, formerly East Boston Neighborhood Health Center, is a large community health center serving the greater Boston area with a commitment to patient and community health. Every role is vital in advancing medicine and delivering the best care experience. This full-time position in Pediatrics is responsible for providing individual and group clinical support and case management services for CATCH program patients. CATCH is NeighborHealth’s care coordination program for medically complex children. The CATCH Social Worker (SW) is an integral member of the CATCH team, conducting clinical assessments, formulating treatment plans, and coordinating interventions with other providers to support participants’ mental health. The SW reports to the Director of Pediatric Complex Care with clinical oversight by Behavioral Health Department Leadership and participates in Behavioral Health Department activities.

Responsibilities

  • Intake and enrollment of new participants.
  • Administer a complete family-centered psychosocial assessment and identify factors impacting the participant’s current level of functioning.
  • Complete initial assessments and contribute to the plan of care by assisting the patient and family to articulate their care priorities and goals.
  • Address families’ role in care for persons with disabilities.
  • Respond appropriately and initiate access to concrete services such as SSI, housing, food stamps, and money management.
  • Serve as a participant and family advocate by clarifying and representing the participant’s voice and perspective to the team and other community providers.
  • Coordinate and demonstrate a leadership role in family meetings.
  • Provide education to patient and family about their rights as patients, navigating healthcare systems, and empowering patients and families as self-advocates.
  • Initiate and refer participants and families to appropriate community agencies.
  • Participate in MassHealth’s CARES program enrollment process, identify goals of care, and participate in CARES team meetings.
  • Coordinate transition from pediatric to adult primary care for patients as they age out of CATCH.
  • Support families through the guardianship process, including explanation, patient assessment, completing applications, and communicating with other providers, schools, and psychologists.
  • Coordinate and assist patients with transition from Early Intervention (EI) to public school, guiding families through school registration and special education evaluations.
  • Annual outreach to patients eligible for school evaluations and those who may need guardianship.
  • Lead parent and patient advisory council, coordinate meetings, and parent/patient engagement opportunities.
  • Address behavioral health needs of CATCH patients, including psychotherapy, family therapy, supportive counseling, and psycho-education.
  • Conduct and present findings in the use of assessment tools.
  • Facilitate groups for parents of newly diagnosed patients with ASD, transition planning for teens and young adults, grief counseling for siblings of children with special needs, and support for financial and social stressors.
  • Assess and treat behavioral/mental health disorders among family members of affected individuals.
  • Maintain appropriate records and documentation in accordance with departmental standards, demonstrating fluency in the use of the electronic medical record.
  • Write progress notes in S.O.A.P. format.
  • Participate actively as part of the CATCH team, including team meetings and assessments.
  • Write and follow up on stated treatment interventions.
  • Support and consult with the CATCH team related to the guardianship process.
  • Assist families in palliative and end-of-life care through supportive counseling and psycho-education.
  • Participate and coordinate discussions regarding advance directives, healthcare proxies, and end-of-life issues.
  • Provide consultation to members of the CATCH team to enhance their understanding of the psychological aspects of care.
  • Encourage effective relationships among staff geared toward building and maintaining a cohesive team.
  • Collaborate and communicate with specialists, behavioral health, and educators as needed.
  • Perform crisis intervention.
  • Develop and lead activities demonstrating clinical expertise, such as group counseling and resource management.
  • Present cases at individual and group supervision sessions.
  • Engage in learning new skills and developing competencies.
  • Demonstrate commitment to a holistic approach to care.
  • Communicate effectively and respectfully through verbal and written skills.
  • Document in accordance with protocol.
