The Licensed Social Worker is responsible for implementing resident services for residents consistent with regulations and established best practices. The Social Worker provides support through resident/family advocacy, ongoing assessment and evaluation, service coordination and care planning. The primary focus of this position is the quality of life of residents to assist them in achieving their optimal psychosocial, emotional and behavioral well-being. This position reports to the Campus Director of Social Services and be covering our long term care facility.

Requirements

  • Bachelor’s degree in Social Work, Gerontology, or related field required.
  • Licensed by the MN Board of Social Work and maintain unrestricted licensure.
  • One or more years of social work experience preferred, preferably in a long-term care environment.
  • One or more years of social work experience preferred, in a long-term care setting for Transitional Care Unit (TCU).

  • An equivalent combination of professional experience and education considered at discretion of management.
