About The Position

The University of Rochester is seeking a Social Worker Lead for its General Ambulatory service. This role involves functioning as a Team Leader for the service and serving as the primary therapist for individuals aged 18 and older who have severe and persistent mental illness. The Team Leader will provide supervision to an interdisciplinary group of clinical staff and may collaborate in policy and program development, operations management, and planning program activities. Additionally, the social worker is an integral member of the Clinic, providing comprehensive mental health services, developing client-driven goals, and offering direct clinical services including individual, family, and group therapy, crisis intervention, case management, and community outreach.

Requirements

Master's degree from an accredited school of social work required.
3 years of relevant experience required.
Licensure as a social worker, as defined by the State Board for Social Work, State Education Department, Division of Professional Licensing Services is required.
If licensure has not been obtained upon employment, the individual has 18 months within which to obtain it, unless duties will be performed under OMH then minimum of a limited permit (not to exceed 1 year) or NYS Licensure will be required upon employment start.

Responsibilities

Performs screening, diagnostic formulation, evaluation, and psychosocial assessments of patients and their families.
Provides social work and behavioral health treatment and education to patients and families through individual, family, and/or group therapy.
Coordinates comprehensive treatment and discharge planning for assigned patients.
Participates as a member of an interdisciplinary treatment team, involving consultation, psychosocial diagnosis, treatment planning, and periodic care review.
Functions as Team Leader, potentially assisting in clinical supervision and contributing to staff evaluations.
Assists in onboarding new therapists, including policies, procedures, clinical teaching, and orientation.
Provides student/internship supervision as appropriate.
Manages interdisciplinary treatment team meetings, facilitating discussions and documenting minutes.
Collaborates with collateral contacts and provides linkages to community resources.
Coordinates with community and SMH resources to ensure continuity of care.
Provides back-up to other units/programs as needed.
Documents treatment in the patient’s electronic medical record in compliance with policy.
Contributes to a culturally competent environment by treating patients, families, trainees, and co-workers sensitively.
Participates in cultural competency events and training.
Attends Social Work Division, Social Work in Psychiatry, and Program staff meetings.
Participates in the activities of the Social Work and Psychiatry Division, ensuring policies and procedures are implemented.
Documents all interventions and contacts in the patient's chart.
Participates in routine committee processes of the Social Work Division.
Facilitates Division and Hospital quality assurance and staff development activities.
Provides coverages and accessibility for intakes, vacations, and sick calls.
Supervises social work or other master’s level graduate students based on experience and supervisor determination.
May offer learning experiences for psychiatric residents, medical students, and family medicine residents upon invitation.
Supervision relates to psychosocial assessment and intervention with patients and their families.
May participate in interdisciplinary research based on experience/interest.
