The Social Worker assigned to the Community Ties Team, part of Strong Ties Clinic, functions as a Team Leader and primary therapist to individuals ages 16 and older who are severely and persistently mentally ill. As a Team Leader, they provide clinical supervision to an interdisciplinary group of staff including social workers, LMHC’s, LMFT’s, and/or Credentialed Alcohol and Substance Abuse (CASAC) counselors. The social worker is an integral member of the Clinic, providing comprehensive mental health services to individuals, ages 16 or older, who have serious and persistent mental illness. In this role they provide psychosocial assessment and treatment to individuals through the development of client-driven goals, primarily based in the community. The social worker provides direct clinical services including individual, family and group therapy, crisis intervention, case management, and community outreach through their participation on an interdisciplinary team. Additionally, the team leader assists with the management of patients assigned to the team related to the coordination of community visits by team members.

Requirements

  • Master’s Degree in Social Work from an accredited social work graduate program required
  • 3 years of post-graduate clinical experience working with individuals with chronic mental illness and/or community mental health preferred
  • Knowledge of mental illness preferred
  • Ability to engage in person centered treatment preferred
  • Knowledge/skills in various therapeutic interventions including CBT, DBT, motivational interviewing preferred
  • Knowledge of risk evaluation and mitigation preferred
  • Clean driving record preferred
  • Previous supervision experience preferred
  • Master's degree from an accredited school of social work.
  • Licensure as a social worker, as defined by the State Board for Social Work, State Education Department, Division of Professional Licensing Services is required.
  • If licensure has not been obtained upon employment, the individual has 18 months within which to obtain it, unless duties will be performed under OMH then minimum of a limited permit (not to exceed 1 year) or NYS Licensure will be required upon employment start.

Responsibilities

  • Functions as Team Leader and provides supervision and evaluation of an interdisciplinary group of staff including social workers, LMHC’s, LMFT’s, and CASAC’s
  • Provides direct clinical services to assigned patients, which may involve individual, family, and/or group therapy, crisis intervention, and diagnostic evaluations.
  • Documents interventions, both direct and indirect, in the patient’s medical record in compliance with the Division, Department, and Hospital policy.
  • Participates as a member of the interdisciplinary treatment team involving consultation, treatment planning, and periodic care review.
  • Collaborates with family, social supports, community agencies (past and present) to ensure appropriate and effective care.
  • Coordinates referrals and facilitates linkage to other appropriate agencies/supports.
  • Provides oversight and monitoring of cases assigned to the Community Ties team including coordination of visits by team members and monitoring of clinical services provided by the team
  • Develops and leads groups for patients (and possibly family members or their support network) which focuses on issues and needs of the severely and persistently mentally ill population
  • Attends Division, Department, and Clinic level staff meetings.
  • Ensures that Division, Department, and Clinic policies and procedures are implemented with regard to professional practice and patient care.
  • Ensures timely completion of statistical reports/forms, time sheets, and other Hospital or Division mandatory in-service trainings and Health Assessments.
  • Subscribes to maintaining a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.
  • Participates in available cultural competency events and training appropriate to job duties.
