About The Position

The Palliative Care Social Worker works collaboratively on an interdisciplinary consulting team to support patients and families dealing with serious illness. The Palliative Care Social Worker is responsible for providing a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, crisis intervention, resource referral, short term counseling, grief support, advance care planning, facilitation of family meetings in collaboration with interdisciplinary team. The Palliative Care Social Worker assumes a leadership role to facilitate the achievement of patient goals and desired clinical, financial, and resource outcomes. Requirements

• Master's Degree in Social Work (MSW or related field)
• California Clinical Social Worker License Upon hire
• 2 years social services experience with a health care related organization.

Nice To Haves

• 3 years experience in a multi-disciplinary health care setting
• Specialized training in palliative care and/or hospice.

Responsibilities
