The Palliative Care Social Worker works collaboratively on an interdisciplinary consulting team to support patients and families dealing with serious illness. The Palliative Care Social Worker is responsible for providing a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, crisis intervention, resource referral, short term counseling, grief support, advance care planning, facilitation of family meetings in collaboration with interdisciplinary team. The Palliative Care Social Worker assumes a leadership role to facilitate the achievement of patient goals and desired clinical, financial, and resource outcomes.
