About The Position

Christ Community Health Services is seeking a compassionate, mission-driven Licensed Clinical Social Worker (LCSW) to join our integrated Behavioral Health team. The LCSW will assess and treat psychological symptoms and the psychosocial aspects of chronic medical conditions among children, adolescents, adults, and older adults. This position will be based at our Third Street Health Center three days per week and will provide mobile outreach services during the remaining two days. The ideal candidate will be comfortable delivering care in both traditional clinical settings and community-based environments. Requirements

  • Master’s degree in Social Work from an accredited institution.
  • Active Tennessee Licensed Clinical Social Worker license or eligibility to obtain Tennessee licensure.
  • At least two years of post-graduate experience, preferably in a hospital, primary care, community health, or other integrated healthcare setting.
  • Experience working collaboratively with physicians and other healthcare professionals to provide team-based patient care.
  • Experience delivering evidence-based interventions for mental health conditions and medical conditions with psychosocial components.
  • Experience using electronic medical records for clinical documentation and interdisciplinary communication.
  • Ability and willingness to provide services in both clinical and mobile outreach settings.
  • Strong assessment, crisis-intervention, communication, and clinical documentation skills.
  • Commitment to patient-centered, culturally responsive, and team-based care.

Nice To Haves

  • Experience working in an FQHC or with underserved populations is preferred.

Responsibilities

  • Provide behavioral health services at the Third Street Health Center three days per week and through mobile outreach services two days per week.
  • Use brief psychological screening tools and clinical interviewing techniques to complete comprehensive assessments.
  • Diagnose behavioral health conditions and develop individualized treatment plans.
  • Evaluate patients experiencing crisis situations, including suicidal or homicidal ideation, and coordinate appropriate interventions and referrals.
  • Deliver evidence-based, focused interventions for common mental health conditions and chronic medical conditions with psychosocial components.
  • Facilitate cognitive-behavioral and psychoeducational groups.
  • Refer patients to psychiatry, spiritual health, and community-based resources when appropriate.
  • Provide timely patient care and communicate promptly with referring providers and other members of the interdisciplinary care team.
  • Collaborate with physicians, dentists, nutritionists, spiritual health advisors, and other healthcare professionals to support whole-person care.
  • Educate medical staff regarding behavioral health standards, psychosocial factors affecting chronic illness, and ethical behavioral health practices.
  • Participate in meetings, trainings, team huddles, case conferences, and other activities that support CCHS’s integrated healthcare model.
  • Monitor behavioral health services, identify concerns affecting patient care, and recommend program improvements.
  • Maintain accurate and timely documentation in the electronic medical record.
  • Provide clinical supervision to graduate practicum students or interns as CCHS social work training programs are developed.
  • Perform other duties as assigned.
