About The Position

The Licensed Bachelor Social Worker (LBSW) reports to the Director of Behavioral Health and is responsible for providing social work services, case management, care coordination, patient education, and resource linkage to patients of Franklin Primary Health Center, Inc. The LBSW collaborates with providers and the interdisciplinary care team to address patients' psychosocial needs, improve access to community resources, reduce barriers to care, and promote positive health outcomes. Clinical counseling and psychotherapy services requiring independent clinical licensure are not performed by this position.

Requirements

  • Bachelor of Social Work (BSW) from a Council on Social Work Education (CSWE) accredited program required.
  • Current Licensed Bachelor Social Worker (LBSW) license in the State of Alabama required.
  • Current Alabama driver's license required.
  • Requires prolonged sitting, standing, walking, bending, reaching, lifting up to 20 pounds, and occasional lifting up to 30 pounds.
  • Requires visual, auditory, and communication skills necessary to perform job duties.
  • Ability to work under stressful conditions while maintaining professionalism.

Responsibilities

  • Conduct psychosocial assessments to identify patient needs, barriers to care, and available support systems.
  • Provide case management and care coordination for patients with medical, behavioral health, and social service needs.
  • Assist patients with obtaining community resources including housing, transportation, food assistance, financial assistance, medication assistance, and other supportive services.
  • Coordinate referrals to community agencies and monitor follow-up to ensure continuity of care.
  • Assist providers with discharge planning, transitional care, and coordination of services.
  • Educate patients and families regarding available resources, benefits, and community programs.
  • Collaborate with physicians, behavioral health providers, nursing staff, and other interdisciplinary team members to develop patient-centered care plans.
  • Maintain accurate, timely, and complete documentation within the electronic medical record.
  • Monitor assigned caseloads and follow up with patients regarding referrals and treatment recommendations.
  • Participate in quality improvement initiatives, population health activities, and care management programs.
  • Assist with grant-funded program requirements, data collection, and reporting as assigned.
  • Participate in outreach activities, health fairs, and community education events.
  • Attend departmental meetings, staff meetings, continuing education, and required in-service training.
  • Maintain patient confidentiality and comply with all HIPAA regulations.
  • Perform other duties as assigned by the Director of Behavioral Health.

  • Maintain a valid Alabama driver's license and proof of automobile insurance if travel is required.
  • Maintain compliance with HIPAA Privacy and Security Rules.
  • Successfully complete required HIPAA training.
  • Protect all patient health information according to federal and organizational policies.
