About The Position
Social Work Services at the Mount Sinai Hospital provides comprehensive social work services and programming to meet the needs of diverse populations. The department believes that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the community on both micro and macro levels, including direct care and prevention. The Department of Social Work Services at The Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in this department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development, and extra-curricular opportunities.
Requirements
- NYS LMSW/LCSW.
- Excellent clinical, collaborative, and organizational skills.
- Experience in providing psychosocial services to patients with acute and chronic illnesses.
- Ability to develop and implement safe, appropriate, and timely discharge plans in a demanding, fast-paced environment.
- Ability to take the lead in coordinating multiple disciplines for complex family meetings.
- Knowledge of crisis, family, and systems theories.
Nice To Haves
- Inpatient hospital experience preferred.
- Internal candidates: Minimum one year in current position unless a Float in Medicine.
Responsibilities
- Providing psychosocial services with an emphasis on complex discharge planning for patients on 9 East (medicine, liver medicine, gastrointestinal diseases requiring surgical intervention, G.I. cancers, and liver disease).
- Providing a range of psychosocial interventions, including support for patients and families coping with the impact of acute and chronic illness, linkage to substance abuse treatment resources, and end-of-life planning.
- Collaborating as a member of the interdisciplinary team.
- Participating in hospital and unit-specific initiatives.
- Participating in program development and implementation.
- Providing coverage for other medicine/surgery units.
