Requirements

Master's degree in Social Work (MSW) from an accredited school of social work.
Current Florida LCSW or equivalent licensure.
Minimum of two (2) years of experience in a hospital or healthcare setting.
Knowledge of community resources and social service programs.
Excellent communication, interpersonal, and problem-solving skills.
Ability to work independently and as part of a multidisciplinary team.

Nice To Haves

Experience with discharge planning and case management.
Familiarity with electronic health record (EHR) systems.

Responsibilities

Assesses psychosocial needs and provides services that enhance patient functioning, emotional well-being and continued access to services and resources in the community
Provides discharge planning, arranges home health and DME, provides counseling, assists with placement/transfers, provides referrals to community services
Assists with utilization review activities.
