The Social Services II role involves providing emotional support, crisis intervention, and counseling for Pediatrics, Adolescents, Adults, Older Adults, and their significant others to help them cope with social and emotional problems. This includes information and referral services, and referrals for financial assistance and supportive services to patients and families dealing with issues related to hospitalization, illness, disability, or trauma. The position also entails case management & milieu management, completing psychosocial assessments, assisting in the development and re-evaluation of Treatment Plans, working as a member of a multidisciplinary team, and coordinating discharge planning. Necessary documentation and maintaining clinical records for each client during their length of stay are also part of the role.
