About The Position
Baptist Health is seeking a Master's Level Social Worker to join their team. This is a Full Time Days opportunity working at the Baptist Metro campus. This Social Worker will be part of an interdisciplinary care team with the goal to enhance well being by helping meet the needs impacting a person's life through psychosocial support and connection to appropriate resources. There is a focus on alleviating barriers and lessening the burden of medical conditions.
Requirements
- 1-2 years Social Work Experience
- Home Health Experience
- A Master's degree in Social Work is required
Responsibilities
- Assessing psychosocial needs and provides services that enhance patient functioning, emotional well-being and continued access to services and resources in the community.
- Providing care coordination
- Providing psychoeducation
- Providing referrals to community services
