About The Position

Baptist Health is seeking a dedicated and compassionate Master’s Level Social Worker (Social Worker II) to join their team. This is a Full-Time, Day Shift opportunity working at their Baptist Downtown campus. In this role, the social worker will be a key member of an interdisciplinary care team, working to improve overall well-being by addressing the psychosocial needs that impact patients' lives. The focus is on reducing barriers to care and easing the burden of medical conditions through emotional support, education, and resource connection.

Requirements

• Master of Social Work required
• 3-5 Years Discharge Planning Experience
• 3-5 Years Case Management Experience

Nice To Haves

• Doctorate of Arts/Science preferred
• Licensed Clinical Social Worker preferred

Responsibilities

• Assess psychosocial needs and deliver services to enhance patient functioning and emotional well-being
• Coordinate care to ensure continued access to support and services
• Provide psychoeducation to patients and families
• Connect individuals with appropriate community resources and referrals
