About The Position

This is a full-time position with 8-hour shifts available during the day. The role is located at Harlingen Medical Center in Harlingen, Texas. The Social Worker II will be responsible for conducting psychosocial assessments, coordinating discharge planning, providing crisis intervention, offering patient and family support, and connecting patients with necessary resources.

Requirements

Master's Degree in Social Work related field OR Master's Degree in Marriage and Family Therapy or related field.
In states without required licensure, current registration with State Board is required within 3 months of hire.
In states with required licensure, license required at time of hire and must be maintained.
Behavioral Violence Prevention Training within 3 months of hire and maintain current.
Minimum one (1) year hospital or Outpatient experience with emphasis in mental health care.
Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and child care, substance abuse, disability, chronic illness, etc.).

Nice To Haves

Current BCLS (AHA) certificate upon hire and maintain current.

Responsibilities

Conduct psychosocial assessments to evaluate emotional, social, and environmental needs affecting patient recovery and care.
Coordinate safe and appropriate discharge plans, including referrals to external services.
Provide crisis intervention during emergencies such as trauma, end-of-life decisions, abuse, or mental health crises.
Offer counseling, education, and advocacy to patients and their families to help them cope with illness and stress.
Link patients to community resources for financial assistance, housing, transportation, mental health services, and more.
