About The Position

This role provides emotional support, crisis intervention, and counseling for various age groups and their significant others to help them cope with social and emotional problems related to hospitalization, illness, disability, or trauma. The position involves providing information and referral services, assisting with financial assistance referrals, case management, milieu management, completing psychosocial assessments, contributing to treatment plan development and re-evaluation, and coordinating discharge planning. The Social Worker will also complete necessary documentation and maintain client clinical records.

Requirements

Master’s Degree in Social Work related field OR Master’s Degree in Marriage and Family Therapy or related field.
LSW (Licensed Social Worker) or equivalent licensure/registration as required by the state.
Minimum one (1) year hospital or Outpatient experience with emphasis in mental health care.
Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and child care, substance abuse, disability, chronic illness, etc.).
Acute Care Hospital Experience

Current BCLS certificate upon hire and maintain current preferred.

Responsibilities

Assist patients and families with social and emotional problems related to hospitalization, illness, disability, or trauma.
Provide information and referral services.
Make referrals for financial assistance and supportive services.
Provide case management and milieu management.
Complete psychosocial assessments.
Assist in the development and re-evaluation of Treatment Plans.
Work as a member of a multidisciplinary team.
Coordinate discharge planning.
Complete necessary documentation and maintain clinical records for each client.
