About The Position

This is a grant-funded position under general supervision providing professional social counseling, support, and clinical case management to high-risk populations in Hamilton County. The work supports prevention, rehabilitation, and self-sufficiency programs. The employee provides guidance, advice, and treatment informed by social work theory. The role assesses clients’ social functioning, developmental status, and ACE history or status; provides individual or group education and counseling; and works with clients to develop plans to improve social functioning. The Social Worker coordinates care and referrals, helps clients access public and private services and community resources, and follows up on referrals and client actions. The position also contributes to interdisciplinary treatment planning, maintains accurate service documentation and community-resource relationships, and may conduct outreach, mentor clients, evaluate educational materials, collect data, and file reports. The position requires in-person work and regular, punctual attendance. It is considered essential during inclement weather, so the employee may need to report to or remain at work during administrative closings. Work may involve home visits, work after regular business hours in high-crime areas, various work sites, presentations, and community meetings. Holiday or weekend work, irregular hours, shift work, and short-notice extensions of shift hours may be required. Overtime, if offered, is not guaranteed.
