About The Position

In support of Kennedy Krieger Institute’s mission, vision, and values, the Social Worker II provides clinical social work services to children, adolescents, adults, and families within our outpatient medical clinics. In this role, you will assess psychosocial needs, develop care plans, provide crisis intervention, connect patients and families with essential resources, and help overcome barriers to health and well-being. The Social Worker II also responds to urgent mental health needs, including positive suicide screenings, and collaborates closely with interdisciplinary teams to ensure coordinated, patient- and family-centered care. This is an excellent opportunity for an LMSW who is passionate about combining clinical social work, crisis intervention, care coordination, and advocacy in a collaborative healthcare environment.

Requirements

  • Current Licensed Master Social Worker (LMSW) license issued by the Maryland Board of Social Work Examiners is required.
  • Clinical social work experience obtained through internships, field placements, or employment is required.
  • Master’s degree in Social Work from an accredited school of social work is required.
  • Comfortable working in a fast-paced, interdisciplinary medical setting and responding to both routine and urgent psychosocial needs.
  • Strong clinical assessment, crisis intervention, care coordination, communication, and resource-navigation skills are important for success in this role.

  • Experience working with children, adolescents, or young adults with developmental, learning, and/or emotional disabilities is preferred.

Responsibilities

  • Accept referrals for care coordination services within outpatient medical clinics.
  • Assess patient and family needs, document contacts and case activity, and coordinate appropriate follow-up.
  • Provide on-call social work support to outpatient medical clinics, prioritizing requests based on acuity.
  • Conduct risk assessments, safety planning, disposition support, and short-term follow-up for patients who screen positive for suicide risk or express suicidal ideation.
  • Connect patients and families with resources related to urgent psychosocial needs, including housing instability, food insecurity, intimate partner violence, and caregiver mental health needs.
  • Coordinate referrals for mental health services and other community-based supports.
  • Provide consultation to outpatient clinicians regarding abuse and neglect reporting.
  • Provide billable clinical sessions for select cases and maintain timely, appropriate documentation in accordance with professional, departmental, and Institute guidelines.
  • Collaborate with medical clinic staff to maintain current resource and referral information.
  • Participate in quality improvement initiatives, committees, training, and community outreach as assigned.
