This role focuses on transitions in care for assigned patient populations, providing screening, assessment, planning, problem resolution, resource management, social work services, counseling, and crisis management related to healthcare and discharge planning needs. The Social Worker manages and collaborates in discharge planning for patients with simple and complex needs, coordinating with post-hospital care agencies. They function as an interdisciplinary team member, collaborating with other healthcare professionals to assess patient/family needs, develop comprehensive care plans, and resolve patient needs. Services are provided in accordance with accepted standards of professional practice and organizational policies and procedures.
