About The Position

This role focuses on transitions in care for assigned patient populations, providing screening, assessment, planning, problem resolution, resource management, social work services, counseling, and crisis management related to healthcare and discharge planning needs. The Social Worker II manages and collaborates in discharge planning for patients with simple and complex needs, coordinating with agencies for post-hospital care. They function as an interdisciplinary health care team member, collaborating with other professionals to assess patient/family needs, develop comprehensive plans of care, and facilitate the resolution of patient needs. Services are provided in accordance with accepted standards of professional practice and organizational policies and procedures.

Requirements

  • Master's degree in Social Work
  • One (1) year experience in a healthcare setting preferred
  • LMSW (Licensed Masters Social Worker) or LISW (Licensed Independent Social Worker)
  • Valid driver’s license
  • Acceptable motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart
  • Proof of auto insurance
  • Knowledge of office equipment
  • Computer skills (word processing)

Responsibilities

  • Screens patients using high-risk indicators such as frequent hospitalization, inadequate support systems, readmissions, homelessness, abuse, neglect, exploitation, and domestic violence.
  • Assesses immediate discharge planning needs and develops and coordinates a discharge plan based on the patient's medical condition, level of independence, functional capacity, family/social support systems, and financial resources.
  • Assesses long-term and/or future patient care needs by identifying probable changes in the level of independence or functional capacity in collaboration with the patient/family and interdisciplinary care team.
  • Provides information and education on community resources to empower patient/family decision-making.
  • Communicates the discharge plan to the patient/family and interdisciplinary care team through timely documentation in the medical record, active participation in discharge rounds, and/or verbal dialogue.
  • Collaborates with the patient/family to develop the discharge plan, ensuring documentation is legible and conforms to hospital and department policies and procedures, including screening, initial discharge plan, weekly status updates, and changes in the plan.
  • Assesses the patient's behavioral health needs in collaboration with physicians, psychiatrists, and other members of the mental health team.
  • Provides counseling, crisis intervention, and psychosocial assessments to patients/families with appropriate medical staff supervision.
  • Collaborates with internal and external resources to develop short-term and long-term plans of care as appropriate.
  • Assumes a leadership role in facilitating meetings and comprehensive care planning with the interdisciplinary team when necessary.
  • Provides clinical oversight for graduate-level MSW students.
  • Collaborates with the department's leadership team in assessing department needs and designing systems to meet department and organizational strategic initiatives.
  • Performs other duties as assigned.
