About The Position

ChristianaCare is hiring a full time Social Worker II for our Primary Care at Home program for New Castle County, DE. As a Primary Care at Home Social Worker, you will play a vital role in supporting patients with chronic and complex medical needs in the comfort of their own homes. This position provides compassionate counseling, care planning, and ongoing support while addressing social determinants of health and helping patients and families navigate challenges. Working closely with an interdisciplinary care team, you will promote patient stability and independence, support care transitions, and connect individuals to community resources and services that enhance quality of life. Requirements

  • Master’s degree in social work (MSW) from an accredited graduate school of Social Work
  • Minimum of 1-2 years of related work experience preferred.
  • Delaware Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) preferred.

Responsibilities

  • Identifying social, emotional, and environmental needs and connecting them with appropriate resources and services.
  • Providing a full spectrum of social work and discharge planning services.
  • Delivering comprehensive case management for patients with complex medical, geriatric, or behavioral health needs.
  • Ensuring patients receive coordinated, compassionate support that promotes stability, independence, and overall well-being.
