About The Position

This full-time position in the Emergency Services department at Prime Healthcare Saint Joseph Medical Center in Joliet, IL, requires a Master's Degree and LSW (LCSW preferred) with at least 1 year of previous hospital experience. The role involves providing emotional support, crisis intervention, and counseling to a diverse patient population, including Pediatrics, Adolescents, Adults, and Older Adults, and their significant others. Responsibilities include information and referral services, referrals for financial assistance and supportive services, case management, milieu management, completing psychosocial assessments, assisting in treatment plan development and re-evaluation, working as part of a multidisciplinary team, and coordinating discharge planning. The role also requires completing necessary documentation and maintaining clinical records for each client.

Requirements

  • Master’s Degree in Social Work related field OR Master’s Degree in Marriage and Family Therapy or related field.
  • In states without required licensure, then current registration with State Board is required within 3 months of hire. In states with required licensure, license required at time of hire and must be maintained.
  • Minimum one (1) year hospital or Outpatient experience with emphasis in mental health care.
  • Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and child care, substance abuse, disability, chronic illness, etc.).

  • Current BCLS certificate upon hire and maintain current preferred.

Responsibilities

  • Provide information and referral services.
  • Provide referrals for financial assistance and supportive services to patients and families with problems related to hospitalization, illness, disability or trauma.
  • Provide case management & milieu management.
  • Complete psychosocial assessments.
  • Assist in the development and re-evaluation of Treatment Plans.
  • Work as a member of a multidisciplinary team.
  • Coordinate discharge planning.
  • Complete necessary documentation and maintain clinical record for each client during his/her length of stay.
