This position identifies patients who would benefit from Case Management interventions based on an initial screening assessment of discharge needs. The Social Worker II assesses all relevant data and obtains information by interviewing the patient/family and performing an objective evaluation of patient needs. They complete psychosocial assessments in accordance with departmental and professional standards to identify emotional, social, and environmental needs related to diagnosis, illnesses, treatment, and life situations. The role formulates, develops, and implements a psychosocial treatment plan utilizing appropriate social work modalities and interventions, which may include crisis intervention, individual and family therapies, and grief and bereavement counseling. This role initiates appropriate internal and external referrals specific to individual patient needs, maintains current knowledge of available federal, state, and local regulations, and advocates for unmet needs on behalf of patients. The Social Worker II participates in the development of a discharge plan in collaboration with the multidisciplinary team to drive patient progression through the continuum of care. They maintain current knowledge and awareness of payer and reimbursement practices impacting the plan of care, coordinate patient and family care conferences as needed, and identify barriers to efficient and effective management of patient care, seeking strategies to eliminate them. Documentation is completed in the Electronic Medical Record in accordance with departmental reporting standards. The role also maintains working knowledge of payer and reimbursement practices impacting the plan of care and demonstrates the ability to guide the patient and family through an evaluation of their options for post-discharge care.
