About The Position

Vacancy is in the Department of Health and Human Services. This is professional social services to the community through various programs in the Department. The current opening is in Maternal/Child Health whose primary function is to help assess, refer, and link prenatal and postpartum families to improve the health, safety, and overall well-being of families to improve positive birth outcomes and healthy child development. Interviews clients; collects information; assesses information; determines eligibility and may make referrals to other agencies and community support. Offers education and support to promote positive health outcomes. Maintains records and detailed information of cases; verifies information received from clients, and follow up on client's progress. If selected, the incumbent will participate in programs to become a Certified Child Passenger Safety Technician (CPST) and a Certified Community Health Worker within 18 months. Incumbent(s) may perform additional related duties as required. This is a CHPEA Union position. The hours of work are 40 per week. This is a grant-funded position renewable on an annual basis. The City of Hartford is not obligated to fund this position upon termination of funding.

Requirements

  • Bachelor's degree from a four-year college or university, with major course work in social services
  • A minimum of five (5) years of experience in the delivery of social services to individuals and families.
  • Pass a physical examination administered by a City physician
  • Pass a drug and alcohol screening examination
  • Pass a background check
