About The Position

This is a full-time position for a Social Worker I. The role involves demonstrating professional responsibility, complying with policies, and maintaining confidentiality. Key responsibilities include conducting timely patient assessments, developing intervention plans, making appropriate referrals, and documenting patient information. The Social Worker will utilize Social Services documents and the Continuum of Care form for medical record documentation, including assessment, goals, interventions, options provided, and referrals/outcomes. A working knowledge of community resources, financial assistance programs, Advance Directive laws, guardianship/Mental Health commitment processes, and Medicaid/Medicare programs is essential. The role also requires taking initiative in gathering resource information.

Requirements

  • Bachelor’s Degree in Social Work or related field required.
  • Excellent communication, diplomacy, and people skills required.

Nice To Haves

  • Licensed as a Social Worker or Independent Social Worker through Iowa Dept. of Public Health and the State of Illinois preferred.
  • Master’s degree from a graduate school of social work accredited by the Council on Social Work Education which included specialization in clinical practice preferred.
  • One to two years of social work experience in a health-care related setting preferred.

Responsibilities

  • Demonstrates professional responsibility in the role of Social Worker.
  • Complies with personnel policies and hospital safety policies.
  • Maintains confidentiality when interacting with patients, families, personnel and the public.
  • Demonstrates ability to gather timely assessment information including components of home environment situation, support systems, coping skills/stressors, strengths, grief/loss issues, adjustment to illness, identified needs, and goals.
  • Develops and carries-out intervention plan and make appropriate referrals.
  • Documents patient assessment and intervention data utilizing Social Services documents and Continuum of Care form in medical record, which includes assessment, goals, interventions, options provided to pt/family, and referrals/outcome.
  • Demonstrates working knowledge of community resources, financial assistance programs, Advance Directive laws and process, guardianship/Mental Health commitment process, Medicaid/Medicare programs.
  • Takes initiative in gathering resource information.
