Catholic Charities of St. Paul and Minneapolis is looking for caring, compassionate individuals to ensures older adults and individuals with disabilities live and thrive in the home of their choosing! You can make a difference in the quality of life of older adults and individuals with disabilities! Catholic Charities is seeking a self-motivated leader who is compassionate, trauma-informed, and culturally aware to serve as a Team Lead on its Medica team. They will work respectfully with children, adolescents, persons in poverty, and people of diverse racial and cultural backgrounds. They are responsible for case management services, short-term counseling, and assisting clients with access to supportive services, and helping clients set goals for themselves and their families. They will provide ongoing aftercare to clients and their families. They will provide comprehensive counseling to clients of Catholic Charities per program needs and must be able to work effectively with clients with complex psychosocial concerns including mental health issues, unstable housing, unemployment, lack of medical insurance, dysfunctional relationships, and legal concerns. They may conduct support/education groups.
