About The Position

The main purpose of this full-time contractual position is to conduct comprehensive biopsychosocial and functional evaluations of adults with functional disabilities to support their ability to remain in the least restrictive setting and maintain the highest possible level of independence and personal well-being. This position will develop person-centered care plans and provide case management services to assist individuals in obtaining recommended services. This position will coordinate long-term services and supports while serving as a liaison among individuals, caregivers, health care providers, community agencies, and interdisciplinary team members to promote quality care and regulatory compliance.

Requirements

  • Must be licensed as a Graduate Social Worker (on or by June 30, 2018), Master Social Worker (on or after July 1, 2018) or Certified Social Worker by the Maryland State Board of Social Work Examiners prior to appointment to a position in State service.
  • Applicants who do not have a license may also apply pending receipt of the required license.
  • Candidates appointed to positions in this classification may be assigned duties which require the operation of an automobile. Employees assigned such duties will be required to possess an appropriate operator's license valid in the State of Maryland.

  • Experience and/or working knowledge of aging, dementia and person-centered care.
