About The Position

This is a Part-Time position in Outpatient Behavioral Health at Saint Mary of Nazareth Hospital, located at 2233 W Division St, Chicago, IL 60622. The role involves providing emotional support, crisis intervention, counseling, information and referral services, and assistance with financial aid and supportive services to patients and families facing issues related to treatment, illness, disability, or trauma. The Social Worker will also be responsible for case management, milieu management, completing psychosocial assessments, contributing to the development and re-evaluation of Treatment Plans, collaborating within an interdisciplinary team, and coordinating discharge planning. Additionally, the role requires completing necessary documentation and maintaining clinical records for each client.

Requirements

Bachelor’s degree in Social Work, Marriage and Family Therapy or other related field
Current registration as an intern with the Board, as appropriate to education and licensing requirements
Current BLS certificate upon hire and maintain current

Nice To Haves

Masters in Psychology, Social Work, Marriage and Family Therapy, Licensed Professional Clinical Counselor or related field
Minimum one (1) year Inpatient or Outpatient Behavioral Health experience
