About The Position

The purpose of this position is to help achieve the vision of Advocate Home Health Services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the Unit team and as such meeting individual performance expectations and contribution to team performance goals is required. Assesses the home care patient's psycho-social needs as related to their family background and environment as it impacts the medical condition and the prognosis for positive outcome; formulate and implement an effective treatment program to meet those needs; and coordinate with all disciplines regarding the patient's social welfare.

Requirements

  • Masters of Social Work services from an accredited school with licensure or licensed social worker (LSW) or licensed clinical social worker (LCSW).
  • 1-2 years of experience in a clinical setting post licensure preferred.
  • A valid driver’s license issued by the Division of Motor Vehicles
  • Basic knowledge of computer use (i.e. Microsoft Word, email access and use)
  • Ability to learn and use computer based scheduling, and documentation system
  • Ability to communicate professionally both verbally and through written reports with strong documentation skills
  • Good interpersonal skills
  • Good time management and self organization skills
  • Licensure in Illinois.

  • Demonstrates respect and understanding of peers and other clinical disciplines and participates as an effective member of the patient care team to formulate an integrated approach to care following Advocate's MVP of Excellence.
  • Demonstrates behaviors that enhance productivity and an understanding of financial and economic conditions in the health care environment.
