About The Position

Services include: Skilled nursing, skilled therapy, medical social work and home health aide services.

Requirements

• Bachelor's degree in Social Work.
• Current Kansas Social Worker License
• Minimum 1 year Social Work experience in a Health Care setting.
• Auto liability insurance.
• Clean driving record.
• Current driver's license.
• Must be able to successfully pass KDADS criminal record check upon hire.
• Must register with and successfully pass the Family Care Safety Registry upon hire.

Nice To Haves

• Current Clinical Social Worker licensure in state of practice.
• Continual Education: AHA BLS for Healthcare Providers

Responsibilities

• Responsible for assessment and planning patient care in conjunction with other disciplines in home care.
• Provides social work service up to and including discharge for patients requiring those services.
• Assists client in obtaining available community, State or Federal services.
