About The Position

Social Worker Director Grandview Rehab is seeking a compassionate and experienced FT Social Worker to join our skilled nursing facility in Connecticut. This role is responsible for addressing residents’ psychosocial needs, supporting families, and ensuring compliance with state and federal regulations. The ideal candidate will have experience in long-term care and a strong understanding of discharge planning and regulatory requirements.

Requirements

• Master's in Social Work - Required
• Prior experience in a skilled nursing facility or healthcare setting strongly preferred
• Knowledge of Medicare, Medicaid, and long-term care regulations
• Strong communication, organizational, and interpersonal skills
• Must meet Connecticut staffing and regulatory requirements for nursing homes
• Experience working within state compliance guidelines and survey readiness is highly preferred

Nice To Haves

• Bachelor’s Degree in Social Work- preferred

Responsibilities

• Conduct comprehensive psychosocial assessments for all residents
• Develop and implement individualized care plans
• Provide counseling and emotional support to residents and families
• Coordinate discharge planning, including community resources and referrals
• Act as a liaison between residents, families, and the interdisciplinary team
• Ensure compliance with Connecticut Department of Public Health regulations
• Address resident concerns, grievances, and advocate for resident rights
• Maintain accurate and timely documentation
