About The Position

Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area. With a staff of 500 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are "home-bound" and frail. We are currently seeking a Social Worker to join our CDMP/GAIN team.

Requirements

  • Post-secondary degree in Social Work (BSW or MSW)
  • Active member of the Ontario College of Social Workers and Social Service Workers (OCSWSSW) in good standing.
  • Proficiency in current Geriatric screening, assessments, risk stratification and response, care planning, system navigation, psychotherapy and psychosocial counselling techniques for working with diverse individual and group clients as well as their families.
  • Demonstrated experience providing an integrated model of practice and service for clients and families which address health from a holistic perspective (including physical, emotional, social, economic, spiritual and environmental influences on health).
  • CPR/first aid certification
  • Valid Ontario Driver’s License and access to a reliable vehicle or public transportation
  • A valid vulnerable sector police record check completed within one year

Responsibilities

  • In collaboration with the interprofessional team, conduct comprehensive geriatric assessments of clients as a geriatric assessor
  • Develop and implement goals and treatment intervention for complex case situations or specialized problems to inform integrative care plan with GAIN Community Team. This will include referral to community services; assisting clients, their families and caregivers in developing or enhancing coping skills related to the social, emotional needs and challenges associated with aging and illness and care coordination with an inter-professional team.
  • Act as an expert resource to the Interprofessional GAIN Teams by providing consultation and information regarding community resources and/or addressing issues through direct intervention where gaps in services exist or by supporting linkages to specialized services in the areas listed but not limited to: mental health and addictions, caregiver stress, advance care planning, etc.
  • The Social Worker practices in accordance with the Ontario College of Social Workers and Social Service Workers (OCCSWSSW) Professional Standards.
