About The Position

This role involves providing social work services to patients and their families within the acute care setting at Markham Stouffville Hospital, excluding in-patient mental health. The position emphasizes psychosocial support, interdisciplinary problem-solving, and connecting patients and families with necessary resources. The role requires managing a diverse caseload in a fast-paced environment, responding to crises, and demonstrating leadership, integrity, and accountability. Experience in short-term supportive counseling for patients and families facing difficult decisions and end-of-life concerns is considered a valuable asset.

Requirements

  • Master of Social Work
  • Registration with the Ontario College of Certified Social Workers and Social Services Workers
  • Recent related previous social work experience in a hospital environment
  • Understanding of loss/adjustment and the grief process
  • Demonstrated strengths in interpersonal relations, problem-solving, and conflict resolution
  • Demonstrated excellent communication skills
  • Demonstrated commitment to professional development
  • Experience in working with electronic documentation, preferably, Meditech
  • Demonstrated good attendance and performance records with the ability to maintain these same standards
  • Commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required

Responsibilities

  • Completing psychosocial assessments
  • Providing short-term, goal-oriented support and counseling for patients and families dealing with grief, chronic life-altering issues, and end-of-life concerns
  • Collaborating with the health care team, patient, and family to develop patient care plans
  • Providing cross-coverage for other Social Workers within the hospital
  • Consulting with community health care professionals
  • Maintaining up-to-date knowledge of community resources
  • Attending and responding to gaps in service delivery
  • Participating in committees and task forces as appropriate
