About The Position

The Social Worker has responsibility for working as a member of the case management team assigned to coordinate the care and services for assigned patients for an episode of illness or treatment in collaboration with the patient, family, physician, patient care team, and payors. The Social Worker utilizes advanced psychosocial skills to facilitate the coordination of care by using the principles of assessment, planning, intervention, and evaluation. The Social Worker promotes and evaluates the effective utilization of resources using current knowledge, awareness of community services, and assuming a leadership role with the patient care team to achieve optimal clinical and resource outcomes.

Requirements

  • Master’s degree in Social Work (must be completed within 2 years of hire)
  • Current CPR recognition (within 3 months of hire)
  • Knowledge of state and federal funding sources and local community services.
  • Understanding of third-party reimbursement methodologies.
  • Strong crisis intervention skills.
  • Critical thinking and analytical skills.
  • Excellent communication and interpersonal skills to establish a high degree of rapport and professional interaction.
  • Ability to maintain strict confidentiality at all times.

Nice To Haves

  • Experience in acute medical care, case management
  • Two years experience in medical social work or counseling in a medical setting
  • Working knowledge of managed care requirements and hospital information systems

Responsibilities

  • Coordinate the care and services for assigned patients for an episode of illness or treatment in collaboration with the patient, family, physician, patient care team, and payors.
  • Utilize advanced psychosocial skills to facilitate the coordination of care by using the principles of assessment, planning, intervention, and evaluation.
  • Promote and evaluate the effective utilization of resources using current knowledge, awareness of community services.
  • Assume a leadership role with the patient care team to achieve optimal clinical and resource outcomes.
  • Assessment, planning, intervention, and evaluation of psychosocial aspects of patient care including issues impacting compliance/self care related to disease or illness and cultural issues.
  • Maintain and regularly update psychosocial skills and knowledge.
  • Build positive working relationships with physicians and external agencies.
  • Effectively communicate with multiple levels of the organization (e.g., managers, physicians, clinical, and support staff).
  • Fulfill the essential functions of the position.
