About The Position

This role involves coordinating comprehensive care plans from admission to discharge, ensuring a seamless, interdisciplinary approach that puts the patient’s goals at the center of their recovery. The Social Worker will assess and address the complex emotional, psychological, and physical needs of patients, providing critical social work support to help them navigate difficult diagnoses. They will advocate for patient rights and preferences, acting as their voice within the medical system to ensure respectful, personalized care. Additionally, the role includes empowering patients and families through education, connecting them with vital financial, emotional, and community resources, and collaborating daily with leadership and care transition teams to continuously evaluate and adapt care plans.

Requirements

  • Licensed Clinical Social Worker credentialed from the Kansas Behavioral Sciences Regulatory Board obtained prior to hire date or job transfer date, OR Licensed Bachelor Social Worker (BSW), OR Licensed Master Social Worker (MSW) credentialed by the Kansas Behavioral Sciences Regulatory Board.
  • Bachelor's degree in Social Work required.

Nice To Haves

  • Case Manager credentialed from the American Case Management Association (ACMA) preferred.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
  • 3 years of experience preferred.

  • Collaborate daily with leadership and care transition teams to continuously evaluate and adapt care plans, ensuring no patient falls through the cracks.
