Requirements

Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
Social Worker specializing in Clinical Practice credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Responsibilities

Coordinate the overall interdisciplinary plan of care across the continuum from admission to discharge, integrating patient/family preferences and values (LSW required, LCSW preferred).
Monitor patient care through assessments, evaluations, and medical records, acting as a resource for adequate documentation and appropriateness of services.
Advocate for necessary resources, the removal of barriers, and appropriate treatment options for post-discharge care.
Maintain ongoing dialog with supervisors and other health providers to ensure the effective implementation of the health plan.
