Requirements

Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Nice To Haves

Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation preferred.

Responsibilities

Deliver direct patient care by administering physician and nursing orders, implementing care plans, and following established organizational policies.
Monitor and document patient conditions, continuously updating the healthcare team on changes and response to treatment.
Coordinate multidisciplinary care and manage patient discharge planning in collaboration with the healthcare team.
